SURGICAL MYCOTIC ONYCHOPATHOLOGY ASSOCIATED WITH SECONDARY NAIL INCARNATION: COMPLEX TREATMENT, AUTHOR’S VIEWS

The scheme of complex treatment and surgical interventions of chronic onychopathology associated with onychomycosis and nail incarnation are optimized. A retrospective and prospective analysis of clinical features and treatment of 919 patients 5–92 years old for a 10-year period: 503 men and 416 women and study of the morphogenesis of onychodestructive and incarnation aspects of fungal infection, biochemical changes were studied. Affected nails removal performed in patients with fungal lesions, and used local and systemic fungicide therapy. Surgical removal of mycosisaffected nails in these patients is recommended to be performed through of nail bed hyperkeratosis dissection areas and onycholized structures with simultaneous removal of dermatophytoma and subungual hyperkeratosis with ingrowth loci and eponycheal hypergranulations, that determines the postoperative pain reduction (χ2=48,32, p<0,01), improving the quality of life, increases in patients the frequency of complete clinical recovery.
